This notice describes the information collected through the EstiVite marketing website and founding-practice application.

Information you provide

When you apply for early access, we collect the name, email address, business name, team size, current scheduling platform, areas of interest, plan interest, and optional message you submit.

How we use it

We use application information to review pilot fit, understand your current setup, respond to you, and plan onboarding if you move forward. We also collect basic source and campaign information when available so we can understand how people find EstiVite.

Where it goes

Application details are sent to EstiVite’s internal task system so the team can review and follow up. We do not use the application form to collect client treatment records or payment information.

This notice covers the current marketing site and pilot application. Product privacy terms for participating practices will be provided as part of onboarding.
